The Salvation Army

Provides assistance with the following: Rent/mortgage, utilities (gas, electric, water, & sewer), healthcare (non-narcotic medication), steel toe work boots or non-slip shoes, and disaster relief. Assistance is first come, first served while funding is available. Households can receive assistance once per12 months.

The Salvation Army Service Extension Locations:
• Human Services, Inc. (812)522-8718 – Assists Jackson County residents ( non-Crothersville residents of Jackson County).
o Rent/Mortgage
o Utilities: electric, gas, water, and sewer (Can only assist towards the disconnect amount/amount needed to restore service. NO security deposits).
o Non-narcotic medication
o Work boots or non-slip shoes.
o Disaster Relief.
– In the event of a house fire, flood, or any type of natural disaster we can offer additional assistance to the household.

• Crothersville: Toby Ortman (812)793-2512 – Assists Crothersville residents through the First Baptist Church – Crothersville.
o Assistance may vary.

Items required for all assistance given through The Salvation Army:

• Intake Sheet.
• Social Security Cards for all household members.
• ID for all adult completing the Intake form and/or using alternate SSN verification.
• Income documentation for all adults (No income guideline):
o Work One Wage & Voucher/DWD for unemployment.
o Most recent check stub(s).
o Social Security and/or SSI Award letter or most recent bank statement.
o Pension/Retirement Benefit letter (Cannot accept bank statements).
o Temporary disability most recent check stubs or benefit letter.
o Veterans benefit award letter (cannot accept bank statements).
o Self-employed – 1040 and Schedule C Tax documents from most recent year.
o Zero income form (Use for those who have zero income, gaps in their employment, or need to self-declare their income).
o Child Support – 12 months print out.
o TANF – 12 months print out.

Rent Assistance

• Landlord Affidavit

NOTE: The participant is required to pay all but the remaining portion of the rent before The Salvation Army can assist unless the landlord is willing to work with the tenant. Must guarantee 30 days of service with funds. The Salvation Army does not pay towards security deposits.

Mortgage Assistance

• Current mortgage statement.

NOTE: The participant is required to pay all but the remaining portion of the mortgage payment before The Salvation Army can assist. Must guarantee the house will not be forclosed.

Utility Assistance (Electric, gas, water & sewer

• Most recent utility bill.

NOTE: The participant is required to pay all but the remaining portion of the disconnect amount or the amount needed to restore service before The Salvation Army can assist unless the vendor is willing to set up a payment arrangement. Must be able to guarantee 30 days of service with funds. The Salvation Army does NOT pay towards security deposits or any tampering/fraudulent charges.

Assistance with Workboots or Nonslip Shoes

• Training receipt (A receipt that shows how much the item will cost with tax included. Vendors: Wal-Mart & Shoe Sensation).

NOTE: If the item exceeds the amount The Salvation Army can assist with, the participant is required to pay the remaining portion, or no assistance will be issued.

Non-Narcotic Medication Assistance

• Pharmacy Profile (A document that states the exact amount needed for the prescription after insurance if applicable).

NOTE: Cannot assist with narcotics. If the prescription(s) exceed(s) the amount The Salvation Army can assist with, the participant is required to pay the remaining portion, or no assistance will be issued.

Gasoline

• Documentation from doctor listing the scheduled appointment.

NOTE: Can only assist with gasoline for transportation if there is a medical need or family death. (Examples: A doctor’s appointment/procedure/surgery or a funeral out of town for an immediate family member). Salvation Army does NOT pay for vehicle repairs.

Motel Voucher

• Verify there is a legitimate need for the emergency shelter.

NOTE: Can only assist with a one-night stay.

Disaster Relief

• Report from the police or fire department, newspaper article, or pictures. In the event of a house fire or flood or any type of natural disaster we can offer additional assistance to the household and/or households if the situation can be verified.
